1. POST OPERATIVE CARE @ HOME PROGRAM (POCH)
Discharging from General Practice Treatment, Elective or Day Surgery? Does your patient feel they need more time to heal? We offer the following services:

2. PRIORITY RESPONSE SUPPORT
(PRS)
For patients that a health professional has assessed and sent home, yet they remain concerned about patient's condition/situation and deem the patient can wait up to 2 hours to see a Registered Nurse with Emergency and Critical Care Assessment Skills:

Constipation Medications

IVC Complications

Full Nursing Assessment (ABCDE)

We assist the patient in attending hospital

We wait with the patient until Ambulance Service arrives (if deemed priority)

3. REFER ON SERVICE
(ROS)
This service is for hospitals, health services, general practitioners, allied health staff and family or friends who believe the patient/person is in need of a Full Nursing Assessment (ABCDE) and includes gathering information concerning the patient's individual physiological, psychological, sociological, and spiritual needs) and needs to be referred onto a service such as:

St John Ambulance, Disability Facility, Disability Care Home, Residential Aged Care Facility, Residential Care Outreach, Adult HiTH, Mental Health HiTH, Mental Health Emergency Response Line (MHERL) or Perth Children's Hospital HiTH and PAC Services.
​
Mobile Nursing and Health Services will then contact the patient and ask for their consent to undergo a health and wellbeing assessment and if denied will be unable to attend the premises. If for any reason the nursing service believes the patient may be unwell we will contact St John Ambulance for an emergency assessment.

4. WOUND MANAGEMENT SERVICE (WMS)
​Limb ulcers
Pressure injuries
Post-surgical wounds
Pilonidal sinuses or abscesses
Cancerous wounds
Swollen legs
Acute wounds (skin tears, abrasions)

4. GENERAL REFERRAL (REF)
You would like one of your patients or clients to be seen by one of the MobileNHS health professionals. E.g. Social Worker or Registered Nurse.
